A well-woman exam is a preventive evaluation of your overall health, covering physical examination, screening tests, and a conversation about anything affecting your body or your plans. If you have never had one, or it has been long enough that you have forgotten, not knowing what to expect can be its own reason to put it off.
Here is what the visit typically includes.
The Physical Exam Portion
This is the part most people picture, and it is usually the shortest part of the visit. It can include:
- Pelvic examination: a gentle, thorough evaluation to assess vaginal and cervical health
- Pap smear, if indicated: screening for cervical cell changes to detect abnormalities
- Breast examination: a clinical exam to evaluate for changes or concerns in breast tissue
A Pap smear is performed during the pelvic exam and takes only a few minutes. A speculum is gently placed so the cervix can be seen, a small sample of cells is collected, and the sample is sent to a laboratory for evaluation. You may feel mild pressure, but the test is generally quick and well tolerated.
Testing That May Be Added
Beyond the exam itself, several tests may be part of your visit depending on your needs. STD screening is available confidentially, based on your risk factors. Personalized lab testing can assess hormone levels, screen for metabolic health, and give a more complete picture of your overall well-being.
Testing is approached thoughtfully rather than automatically, so what you receive is appropriate for your age, risk factors, and overall health.
The Conversation, Which Is Half the Appointment
A well-woman visit is not only physical. Expect discussion of your menstrual patterns and any hormone-related concerns, your contraception and family planning goals, menopause symptoms and treatment options if relevant, and sexual health and function in a confidential, judgment-free setting.
If contraception is on your list, this is where you review how different methods work, how often they are used, and how effective they are. Options range from IUDs and the Nexplanon implant to pills, the patch, the vaginal ring, the Depo-Provera shot, barrier methods, and permanent options. Pain control methods are available for IUD insertion, so ask your provider if that is a concern.
You will also get guidance on age-appropriate screenings outside the OB/GYN office, such as mammography, bone density testing, colon cancer screening, and genetic testing.
What Can Happen the Same Day
A well-woman visit often resolves more than the exam itself. On-site laboratory testing covers pregnancy testing, urinalysis, and blood work. Point-of-care urinary testing allows rapid evaluation of urinary tract infections and other common concerns.
If a pelvic ultrasound is recommended, it is done on site and results are reviewed with a provider the same day. Pregnancy testing comes with same-day ultrasound and counseling when needed. Some procedures can be performed at the same visit when appropriate, and if a surgical concern comes up, you receive evaluation and guidance covering options, risks, and next steps.
If Your Pap Results Come Back Abnormal
Most results are normal. If something is flagged, additional testing or monitoring may be recommended, and many abnormal findings are minor and resolve on their own. You will be walked through next steps clearly and promptly.
HPV is worth understanding here, because it is extremely common. Most sexually active people are exposed at some point, many clear the virus naturally within one to two years, and often there are no symptoms at all. A positive HPV result is usually not cause for alarm. Depending on your results, the recommendation may be repeat testing, closer monitoring, or further evaluation such as colposcopy. A diagnosis does not reflect your behavior or hygiene. It is simply part of normal exposure.
We can also discuss whether HPV vaccination makes sense for you.
Coming In Prepared
The most useful thing you can bring is a short list of what you have noticed since last year and what you want to ask. Nothing on it is too small or too awkward. Care here is respectful and individualized for patients of all gender identities and sexual orientations.
